The handheld is offline (or the connection is weak, or the shift is moving too fast). The system makes a decision locally &#8212; *this is what happened* &#8212; and business continues on that basis: allocations, replenishment, promises to customers.</p><p>Then the device syncs later.</p><p>Now a centrally-biased ruleset replays the world as if it were clean: dedupe logic, late validations, ERP &#8220;truth&#8221; corrections. The system rewrites yesterday&#8217;s reality with today&#8217;s information.</p><p>What looked like a single inventory movement becomes a cascade:</p><ul><li><p>reservations get churned and re-assigned,</p></li><li><p>replenishment triggers fire (or don&#8217;t) based on rewritten states,</p></li><li><p>pick waves break and operators get sent back into the aisle,</p></li><li><p>stockouts appear on paper while inventory is physically somewhere else,</p></li><li><p>exceptions pile up and humans spend hours reconciling what the system *now* claims happened.</p></li></ul><p>Not because anyone was careless &#8212; because the system treated late truth as if it were the only truth.</p><p>The model didn&#8217;t fail. The missing layer did.</p><p>Because when this happens, most orgs discover they have no control surface:</p><ul><li><p>they can&#8217;t see <strong>exactly</strong> what the system believed at decision time,</p></li><li><p>they can&#8217;t explain why it was allowed,</p></li><li><p>they can&#8217;t override it without side effects,</p></li><li><p>they can&#8217;t rollback cleanly,</p></li><li><p>and they can&#8217;t prove the chain of causality without a forensic dig.</p></li></ul><p>If you can&#8217;t observe, explain, override, rollback, and prove what happened, you don&#8217;t have autonomy &#8212; you have unmanaged risk with a UI.</p><h2>The thesis  - executive version</h2><p>You don&#8217;t need more intelligence &#8212; you need controllability.</p><p>In operations, autonomy is not a model capability. It&#8217;s a <strong>system property</strong>.</p><p>And the limiting factor isn&#8217;t whether an AI can propose the right action. It&#8217;s whether the organization can:</p><ul><li><p>keep decision-making inside explicit boundaries,</p></li><li><p>see and stop failure early,</p></li><li><p>reverse the blast radius,</p></li><li><p>and produce evidence when the system is questioned (internally, by partners, or by regulators).</p></li></ul><p>That layer is what I call a <strong>control surface</strong>. It&#8217;s the operator interface + governance primitives that make automation safe to entrust.</p><p></p><h2>What to fund first  - before you fund &#8220;more autonomy&#8221;</h2><p>If you&#8217;re sponsoring autonomy in a real operation (warehouse, field service, last-mile, infrastructure), the safest question to ask is not &#8220;how accurate is the model?&#8221;</p><p>Ask: <strong>what will we do when the system is wrong, late, or under partial truth?</strong></p><p>Fund these first &#8212; before you fund more autonomy.</p><h3>1) Evidence , so you can explain what happened</h3><p>You need an event trail that makes &#8220;who knew what, when?&#8221; answerable without a forensic dig.</p><p>If inventory drift is your reality, evidence is the difference between <em>reconciling</em> and <em>arguing</em>.</p><p>Example: a mobile device posts a pick confirmation offline at 10:07. The ERP receives it at 11:42. In between, the system made replenishment and reservation decisions based on what it believed at the time. When quantities later &#8220;correct,&#8221; the question isn&#8217;t &#8220;who is wrong?&#8221; &#8212; it&#8217;s <strong>what did the system know when it acted, and what changed after?</strong></p><p>Minimum:</p><ul><li><p>decision event logs (inputs used, what was stale/missing, thresholds)</p></li><li><p>state transitions (before/after)</p></li><li><p>reason codes tied to policy/constraints</p></li><li><p>linkage from decision &#8594; action &#8594; downstream records</p></li></ul><p>A practical test: can an operator (or auditor) reconstruct the timeline in under 5 minutes without asking engineers to dig through logs?</p><h3>2) Exception handling as a first-class product surface</h3><p>Exceptions are not an edge case. They&#8217;re where operations spend money.</p><p>Example: a late sync changes available quantity and the system can&#8217;t reconcile reservations cleanly. If that turns into a generic &#8220;inventory mismatch&#8221; alert, it becomes pure noise. Operators either ignore it (and the mismatch becomes a customer issue later) or they stop trusting the system entirely.</p><p>A governable system routes exceptions like work, not like drama:</p><ul><li><p>classify the exception (what kind of mismatch is this?),</p></li><li><p>assign ownership (who is accountable to resolve it?),</p></li><li><p>set an SLA (how long can we tolerate it?),</p></li><li><p>define a safe default (what happens if nobody touches it?),</p></li><li><p>and capture feedback that improves the policy (so the same exception happens less).</p></li></ul><p>Fund:</p><ul><li><p>an operator queue that supports triage</p></li><li><p>clear ownership and SLAs for exception types</p></li><li><p>safe defaults when nobody responds</p></li><li><p>feedback capture that improves policy (not blame)</p></li></ul><h3>3) Override with authority</h3><p>A &#8220;manual approval step&#8221; is not governance if it&#8217;s a dumpster.</p><p>Example: inventory sync arrives late and the system wants to auto-correct quantities and re-run allocations. If the only override is a generic approval popup, operators will do one of two things:</p><ul><li><p>approve everything to clear the queue (because the shift is moving), or</p></li><li><p>bypass the feature entirely (because it creates work).</p></li></ul><p>A real override is closer to a safety control:</p><ul><li><p>it has <strong>scope</strong> (&#8220;stop *this* adjustment, not the whole warehouse&#8221;),</p></li><li><p>it has <strong>consequences</strong> (&#8220;if vetoed, route to exception type X&#8221;),</p></li><li><p>and it has a <strong>safe default</strong> (what happens if nobody responds).</p></li></ul><p>Fund:</p><ul><li><p>veto power with clear accountability</p></li><li><p>escalation paths when vetoed</p></li><li><p>policy-level constraints (what is never allowed)</p></li><li><p>rate limits / quotas to prevent rubber-stamping</p></li></ul><h3>4) Rollback (or compensation) that&#8217;s actually tested</h3><p>A kill switch without rollback is theatre.</p><p>Inventory is a perfect example because you rarely get a clean &#8220;undo.&#8221;</p><p>If the system auto-posts an adjustment and that adjustment triggers downstream work (replenishment tasks, re-reservations, re-picks), rolling back the original adjustment isn&#8217;t enough &#8212; you need <strong>compensation</strong> that unwinds the operational consequences.</p><p>This is where autonomy projects quietly die: the model can decide, but the organization cannot reverse the blast radius without heroics.</p><p>Fund:</p><ul><li><p>explicit rollback/compensation workflows per action type</p></li><li><p>time windows where rollback is clean</p></li><li><p>ownership of rollback when customer/inventory/finance are impacted</p></li><li><p>idempotency, retries, and dedupe designed as control, not plumbing</p></li></ul><p>A practical gate: for every automated action, write down the rollback path before you ship it. If you can&#8217;t, that workflow is not ready for autonomy.</p><h3>5) Observability that matches operational reality</h3><p>Dashboards are not observability if they hide latency and partial truth.</p><p>Fund:</p><ul><li><p>timing markers (when data was captured vs when it arrived)</p></li><li><p>confidence/uncertainty surfaced in operational terms</p></li><li><p>drift detection (what changed after the decision)</p></li></ul><p><strong>Rule of thumb</strong>: if you can&#8217;t fund evidence + rollback, you can&#8217;t afford autonomy.</p><h2>What not to fund first (common traps)</h2><p>These look productive in slide decks and fail in the field:</p><ul><li><p><strong>&#8220;More intelligence&#8221; without controls</strong> (you scale failure faster)</p></li><li><p><strong>Autonomy in irreversible workflows</strong> (inventory, finance, customer promises) without rollback</p></li><li><p><strong>Manual approvals as a safety blanket </strong>(becomes blame-in-the-loop)</p></li><li><p><strong>A control surface built after the incident</strong> (the worst time to design governance)</p></li></ul><p></p><h2>Minimal reference architecture (so this is buildable)</h2><p>Keep it simple. That turns legitimate edge decisions into retroactive errors.</p><p>If you want governable operations, the edge can&#8217;t be treated as wrong by default.</p><h2><strong>The model that looks like governance</strong></h2><p>Most automation projects start with a clean mental model:</p><ul><li><p>The ERP is the source of truth.</p></li><li><p>The mobile app is a thin execution layer.</p></li><li><p>Offline is an unfortunate gap.</p></li><li><p>When connectivity returns, we reconcile and restore consistency.</p></li></ul><p>On paper, this looks like control, auditability, and safety.</p><p>In practice, it often means this:</p><blockquote><p>The system with the least context at the moment of decision gets the final say.</p></blockquote><p>That&#8217;s how legitimate autonomy gets invalidated&#8212;quietly, systematically, and without ever &#8220;failing&#8221; in the field.</p><div><hr></div><h2><strong>Reality: offline is the operating mode</strong></h2><p>Field work doesn&#8217;t happen in clean conditions:</p><ul><li><p>connectivity is intermittent</p></li><li><p>customers change plans on site</p></li><li><p>substitutions happen because inventory is physical, not a table</p></li><li><p>approvals are bounded authority decisions, not philosophical debates</p></li></ul><p>Offline-first isn&#8217;t a technical feature.</p><p>It&#8217;s a design choice that says: the edge is allowed to operate under constraints.</p><p>That&#8217;s <strong>bounded autonomy</strong>.</p><p>And if it&#8217;s designed correctly&#8212;bounded authority, clear rules, captured evidence&#8212;it can be both useful and safe.</p><p>Until reintegration.</p><div><hr></div><h2><strong>The pattern: the &#8220;Delayed Veto&#8221;</strong></h2><p>Here&#8217;s the pattern I keep seeing:</p><ol><li><p>The edge makes a legitimate decision under constraints.</p></li><li><p>Integration later replays that decision through a centrally-biased ruleset.</p></li><li><p>The system issues corrections that rewrite what already happened.</p></li><li><p>Operators learn workarounds (delay sync, avoid features, shadow ops).</p></li></ol><p>That&#8217;s not governance.</p><p>That&#8217;s a <strong>delayed veto</strong>: the system vetoes decisions after the customer already experienced the outcome.</p><div><hr></div><h2><strong>Failure mode #1: reintegration that reverses legitimate field decisions</strong></h2><p><strong>System(s):</strong> Business Central &#8596; offline-first mobile sales app &#8596; back-office reconciliation</p><p><strong>Assumption:</strong> once data syncs, ERP state should override local decisions to restore consistency.</p><p><strong>Reality:</strong> field decisions were valid&#8212;partial loads, substitutions, bounded judgment under real constraints.</p><p><strong>Failure:</strong> post-sync corrections reversed decisions, triggering credit notes, disputes, and loss of trust.</p><p>Autonomy didn&#8217;t fail in the field.</p><p>It failed at reintegration.</p><p>The system allowed the work to happen&#8212;then punished it later.</p><p>And the lesson operators learn isn&#8217;t &#8220;trust the system.&#8221;</p><p>It&#8217;s &#8220;delay sync&#8221; or &#8220;avoid advanced features.&#8221;</p><p>That&#8217;s how shadow operations emerge.</p><p>And shadow operations are where governance actually dies.</p><h2><strong>Failure mode #2: credit control that punishes completed delivery</strong></h2><p><strong>System(s):</strong> F&amp;O credit management &#8594; offline-first mobile delivery app &#8594; finance posting</p><p><strong>Assumption:</strong> credit limits must be enforced centrally at posting time to control financial risk.</p><p><strong>Reality:</strong> deliveries happened offline to known customers under bounded local authority, urgency, and relationship context.</p><p><strong>Failure:</strong> on sync, delivered orders were blocked or reversed due to exceeded credit limits&#8212;triggering credit notes, invoice disputes, and customer-facing chaos.</p><p>Let&#8217;s be precise: <strong>credit limits are legitimate</strong>. Financial control matters.</p><p>The failure is <em>where</em> and <em>when</em> enforcement happens.</p><p>Enforcing limits only at posting time assumes something that isn&#8217;t true in field operations: that the moment of financial recognition is also the moment of operational decision.</p><p>It isn&#8217;t.</p><p>The risk was accepted earlier&#8212;offline, on the truck, under bounded authority, with full local context.</p><p>Integration didn&#8217;t manage that risk.</p><p>It re-labeled it as an error after the fact.</p><p>The result wasn&#8217;t safety.</p><p>It was rework, disputes, and erosion of trust&#8212;internally and with customers.</p><p>That&#8217;s how autonomy breaks quietly.</p><h2><strong>Integration is not neutral</strong></h2><p>Integration is often treated as plumbing:</p><ul><li><p>move data</p></li><li><p>map fields</p></li><li><p>call APIs</p></li><li><p>reconcile states</p></li></ul><p>But integration is not neutral.</p><p>It decides:</p><ul><li><p>which system gets to be &#8220;truth&#8221;</p></li><li><p>which timestamps win when reality conflicts</p></li><li><p>which actions are reversible</p></li><li><p>who gets blamed when states disagree</p></li></ul><p>If you don&#8217;t design these decisions explicitly, you still get decisions.</p><p>You just get them as accidents.</p><p>And bounded autonomy cannot survive accidental governance.</p><h2><strong>When &#8220;source of truth&#8221; becomes &#8220;source of denial&#8221;</strong></h2><p>Enterprise systems love the phrase <em>single source of truth</em>.</p><p>It&#8217;s useful&#8212;until it becomes dogma.</p><p>In field operations, truth is often negotiated:</p><ul><li><p>finance truth (what can be recognized)</p></li><li><p>field truth (what actually happened)</p></li><li><p>customer truth (what they experienced)</p></li><li><p>partner truth (what can be verified)</p></li></ul><p>When these collide, you have two choices:</p><ol><li><p>Pretend collisions are errors and auto-correct them.</p></li><li><p>Treat collisions as a governed part of the system.</p></li></ol><p>Bounded autonomy breaks in option 1.</p><p>Because you&#8217;re not governing chaos&#8212;you&#8217;re trying to erase it.</p><p>And it comes back as disputes, delays, and workarounds.</p><div><hr></div><h2><strong>Governance-first integration (a stance)</strong></h2><p>If autonomy breaks at reintegration, the fix isn&#8217;t &#8220;better sync.&#8221;</p><p>It&#8217;s governance-first integration.</p><p>This is not a framework. It&#8217;s a stance you apply before funding autonomy.</p><h3><strong>1) Separate transport from authority</strong></h3><p>APIs should move facts.</p><p>Policies decide.</p><p>Receiving a record does not automatically grant the ERP the right to invalidate the decision that produced it.</p><p>Authority must be explicit:</p><ul><li><p>what can be decided offline</p></li><li><p>what must be deferred</p></li><li><p>what is allowed but requires evidence</p></li></ul><h3><strong>2) Make state transitions explicit</strong></h3><p>Most reintegration failures are state-transition failures.</p><p>If &#8220;order created offline&#8221; That work matters. But capability is not the gating factor once you move from pilots to production.</p><p><strong>The gating factor is controllability</strong></p><p>Not the illusion of control through dashboards. Not a kill switch. Not &#8220;human-in-the-loop&#8221; as a checkbox.</p><p>Controllability is the architecture that keeps autonomous deployments <strong>governable, safe, and accountable</strong> as complexity accelerates.</p><p>The question isn&#8217;t &#8220;can it operate?&#8221; <br>It&#8217;s <strong>&#8220;can we govern it when it does?&#8221;</strong></p><h2>Autonomy is a capability. Controllability is the requirement.</h2><p>Here is the shift I wish more autonomy leaders made earlier:</p><ul><li><p><strong>Autonomy</strong> answers: <em>Can the system perform the task?</em></p></li><li><p><strong>Controllability</strong> answers: <em>Can the organization stay in charge of the system at scale?</em></p></li></ul><p>Autonomous deployments become real when they hit the things demos avoid:</p><ul><li><p>degraded connectivity</p></li><li><p>sensor drift</p></li><li><p>ambiguous rules</p></li><li><p>unexpected human behavior</p></li><li><p>conflicting priorities</p></li><li><p>partial failures (not total failures)</p></li><li><p>regulatory and board scrutiny</p></li></ul><p>The models assumes the world is mostly stable and exceptions are rare.</p><blockquote><p></p><p>Reality is the opposite: <strong>exceptions are the product.</strong></p><p></p></blockquote><p>If your autonomous deployment can&#8217;t be <strong>exercised and enforced through exceptions</strong>, it cannot scale. It will either be frozen by risk owners, or it will scale until it breaks trust &#8212; and then get frozen anyway.</p><p></p><h2>The common failure pattern: capability first, governance later</h2><p>Most teams build autonomous deployments in a familiar sequence:</p><ol><li><p>Prove the model works</p></li><li><p>Increase autonomy</p></li><li><p>Scale the deployment</p></li><li><p>Add governance &#8220;when needed&#8221;</p></li></ol><p>This sequence is backwards. Governance can&#8217;t be bolted on after the system has already defined:</p><ul><li><p>what counts as a valid action</p></li><li><p>who has authority to override</p></li><li><p>what evidence is captured</p></li><li><p>how rollbacks work</p></li><li><p>what &#8220;safe mode&#8221; actually means</p></li></ul><p>When governance is an afterthought, failures look mysterious and political:</p><ul><li><p>engineers say the system was &#8220;within expected behavior&#8221;</p></li><li><p>operators say the system was &#8220;unpredictable&#8221;</p></li><li><p>risk says &#8220;pause the rollout&#8221;</p></li><li><p>leadership says &#8220;we&#8217;re not ready for autonomy&#8221;</p></li></ul><p>The technology didn&#8217;t fail. The deployment failed because <strong>control was never designed as a first-class requirement.</strong></p><h2>A practical definition: controllability at scale</h2><p>I use a simple definition:</p><blockquote><p><strong>Controllability at scale</strong> means an autonomous deployment remains observable, explainable, governable, reversible, and auditable as its scope expands.</p></blockquote><p>That&#8217;s still abstract, so let&#8217;s make it operational.</p><p>If you&#8217;re responsible for autonomous deployments (or approving them), you want to know:</p><ul><li><p>Can we see what the system is doing <em>in time to intervene</em>?</p></li><li><p>Can we explain decisions to humans <em>who own consequences</em>?</p></li><li><p>Can we override decisions with clear <em>authority and low friction</em>?</p></li><li><p>Can we roll back behavior safely when <em>something goes wrong</em>?</p></li><li><p>Can we prove <em>what happened after the fact with evidence that stands up to scrutiny</em>?</p></li></ul><p>Those are not &#8220;nice to have&#8221; features.</p><p>They are the difference between a controlled deployment and a liability.</p><div><hr></div><h1>Appendix</h1><h2>The controllability scorecard: 5 tests every autonomous deployment should pass</h2><p>Below is a scorecard I&#8217;ve found useful across autonomous and hybrid human&#8211;machine systems.</p><p>Score each test from <strong>0 to 2</strong>:</p><ul><li><p><strong>0</strong> = missing / informal / only works in ideal conditions</p></li><li><p><strong>1</strong> = partial / manual / works sometimes</p></li><li><p><strong>2 </strong>= designed-in / reliable / exercised regularly</p></li></ul><p>You don&#8217;t need 10/10 to start.</p><p>But if you&#8217;re trying to <em>scale</em> autonomous deployments with a 3/10 controllability posture, you&#8217;re not moving fast. You&#8217;re accumulating <strong>shutdown debt</strong>.</p><h3>Test 1 &#8212; Observe: Can we see state and intent in time to act?</h3><p><strong>What &#8220;pass&#8221; looks like (2/2):</strong></p><ul><li><p>You can see the system&#8217;s <strong>current state</strong>, <strong>active constraints</strong> and <strong>confidence/uncertainty</strong>.</p></li><li><p>Telemetry isn&#8217;t just raw metrics; it&#8217;s operationally meaningful.</p></li><li><p>You can detect when the system is outside its expected operating envelope.</p></li></ul><p><strong>Common failure mode:</strong></p><p>The autonomous deployment &#8220;looks fine&#8221; right until it isn&#8217;t. <br>You get outcomes (late deliveries, near misses, policy violations), but you can&#8217;t see the leading indicators. Operators rely on gut feel and ad-hoc screenshots.</p><p><strong>What to build (practical):</strong></p><ul><li><p>clear state model (what state is the system in?)</p></li><li><p>envelope monitoring (what conditions are allowed?)</p></li><li><p>uncertainty surfaced as a signal (not hidden)</p></li><li><p>event timelines that operators can actually read</p></li></ul><p>Observation is not about having more dashboards.<br>It&#8217;s about being able to answer: <strong>&#8220;What is it doing right now, and is that allowed?&#8221;</strong></p><p></p><h3>Test 2 &#8212; Explain: Can it produce a human-usable rationale?</h3><p>Explainability is commonly treated as a machine learning problem.<br>In practice, it&#8217;s a governance problem.</p><p><strong>What &#8220;pass&#8221; looks like (2/2):<br></strong>For any material action, the system can output:</p><ul><li><p>what it observed</p></li><li><p>what rules/constraints were active</p></li><li><p>what options it considered</p></li><li><p>why it chose one over the others</p></li><li><p>what it would do if conditions changed</p></li></ul><p>Not a research-grade interpretability report.</p><p>A <strong>human-usable decision narrative</strong>.</p><p><strong>Common failure mode:</strong></p><p>After an incident, you can reconstruct a story only by stitching together logs. That&#8217;s not explainability &#8212; that&#8217;s archaeology.</p><p>Worse: different teams reconstruct different stories, and governance turns into a debate about whose narrative is &#8220;true.&#8221;</p><p><strong>What to build (practical):</strong></p><ul><li><p>decision summaries as first-class outputs (&#8220;decision receipts&#8221;)</p></li><li><p>conflict/uncertainty flags (&#8220;this was a low-confidence choice&#8221;)</p></li><li><p>operator-facing explanations, not engineer-facing traces</p></li></ul><p>If you can&#8217;t explain decisions, scaling will be blocked by the people who carry consequences: safety, operations, compliance, boards.</p><p></p><h3>Test 3 &#8212; Override: Can authorized humans (or policies) intervene, cleanly?</h3><p>&#8220;Human-in-the-loop&#8221; is often a slogan. Override is an architecture requirement.</p><p><strong>What &#8220;pass&#8221; looks like (2/2):</strong></p><ul><li><p>Clear authority: who can override what, under which conditions.</p></li><li><p>Low-friction intervention: the system doesn&#8217;t fight the override.</p></li><li><p>Multiple override levels:</p><ul><li><p>immediate stop/hold</p></li><li><p>policy constraint changes</p></li><li><p>reroute / reassign</p></li><li><p>escalation to a human supervisor</p></li></ul></li></ul><p><strong>Common failure mode:<br></strong>Overrides exist but are unusable:</p><ul><li><p>too slow (latency, approvals)</p></li><li><p>too blunt (only &#8220;stop everything&#8221;)</p></li><li><p>too informal (operators text an engineer)</p></li><li><p>too risky (override breaks other systems)</p></li></ul><p>In that world, people will bypass the system. That&#8217;s not safety.<br>That&#8217;s ungoverned autonomy hidden behind a UI.</p><p><strong>What to build (practical):</strong></p><ul><li><p>explicit override pathways (tested, trained, and logged)</p></li><li><p>escalation thresholds (ambiguity, novelty, risk)</p></li><li><p>separation of powers (operator vs supervisor vs safety officer)</p></li></ul><p>A scalable autonomous deployment is <strong>autonomous within constraints</strong> &#8212; and constraints require enforcement plus intervention paths.</p><h3>Test 4 &#8212; Rollback: Can we revert behavior safely and predictably?</h3><p>Most autonomy teams treat rollback as an engineering hygiene issue. At scale, rollback is a governance primitive.</p><p><strong>What &#8220;pass&#8221; looks like (2/2):</strong></p><ul><li><p>You can revert model/config/behavior in minutes, not days.</p></li><li><p>&#8220;Safe mode&#8221; is defined and exercised.</p></li><li><p>You can roll back <strong>partially</strong> (one region, one fleet segment, one workflow).</p></li></ul><p><strong>Common failure mode:</strong></p><p>A deployment goes wrong and your only options are:</p><ul><li><p>keep running and hope it stabilizes</p></li><li><p>shut down everything</p></li></ul><p>Both create organizational trauma. <strong>Trauma kills scaling</strong>.</p><p><strong>What to build (practical):</strong></p><ul><li><p>versioned policies and configs with controlled rollout</p></li><li><p>canary deployments and staged rollout gates</p></li><li><p>rollback playbooks that operators can execute</p></li></ul><p>Rollback is how you turn &#8220;we learned something&#8221; into &#8220;we improved safely.&#8221;</p><h3>Test 5 &#8212; Prove (Evidence): Can we demonstrate what happened to a board or regulator?</h3><p>This is the test many teams ignore until it is too late.</p><p><strong>What &#8220;pass&#8221; looks like (2/2):</strong></p><ul><li><p>You can produce evidence for a specific decision:</p></li><li><p>inputs used (and their quality)</p></li><li><p>constraints active at the time</p></li><li><p>decision record and rationale</p></li><li><p>overrides applied (by whom, when, why)</p></li><li><p>post-incident analysis with traceability</p></li></ul><p><strong>Common failure mode:<br></strong>Your post-incident package is a narrative:</p><ul><li><p>&#8220;the system behaved unexpectedly&#8221;</p></li><li><p>&#8220;we are committed to safety&#8221;</p></li><li><p>&#8220;we will implement improvements&#8221;</p></li></ul><p>That language may be sincere. But it is not evidence. And without evidence, trust does not compound.</p><p><strong>What to build (practical):</strong></p><ul><li><p>decision receipts stored immutably (with access controls)</p></li><li><p>audit-ready timelines</p></li><li><p>clear ownership of evidence production</p></li></ul><p>If you can&#8217;t prove what happened, you can&#8217;t scale autonomous deployments into regulated environments &#8212; and you can&#8217;t defend them when something inevitably goes wrong.</p><div><hr></div><h2>A simple way to use the scorecard</h2><p>If you want this to be actionable, don&#8217;t start by scoring &#8220;autonomy maturity.&#8221;Start by scoring controllability on a specific autonomous deployment.</p><h3>Step 1: Pick one deployment that matters</h3><p>Choose a deployment where the consequences are real:</p><ul><li><p>safety implications</p></li><li><p>customer trust</p></li><li><p>compliance exposure</p></li><li><p>significant cost impact</p></li></ul><h3>Step 2: Score 0&#8211;2 on each test</h3><p>Do it with a cross-functional group:</p><ul><li><p>engineering</p></li><li><p>operations</p></li><li><p>safety / risk</p></li><li><p>(if relevant) compliance</p></li></ul><p>The goal isn&#8217;t consensus by debate. The goal is to surface where control is informal or imaginary.</p><h3>Step 3: Fix the weakest link before scaling</h3><p>Controllability behaves like a chain. <br>If you can observe and explain but can&#8217;t override, you still don&#8217;t have control.  If you can override but can&#8217;t roll back safely, you&#8217;ll avoid overriding. <br>If you can do all of that but can&#8217;t prove what happened, scaling will be blocked by governance and trust.</p><h3>Step 4: Treat &#8220;exceptions&#8221; as design inputs, not embarrassment</h3><p>Every override, escalation, and rollback is a governance signal. <br>If your autonomous deployment needs frequent human intervention, that is not a reason to hide humans.</p><p>It is a reason to:</p><ul><li><p>formalize the intervention pathway</p></li><li><p>tighten constraints</p></li><li><p>improve observation</p></li><li><p>reduce ambiguity</p></li></ul><p>Govern what you automate &#8212; before scale makes it brittle.</p><p></p><h2>Why controllability increases speed (not bureaucracy)</h2><p>Teams often fear that governance will slow them down. Bob knows that the customer at 12 Main St. has a gate code that isn&#8217;t in the CRM. Bob knows which machines need a wrench and which ones need a kick.</p><p>When you automate, you are trying to replace Bob&#8217;s intuition with binary code. But if you don&#8217;t extract Bob&#8217;s knowledge first, your shiny new automation will blindly execute logic that fails in the real world.</p><p>Automation is a force multiplier. If your current process is chaotic&#8212;relying on tribal knowledge and &#8220;pencil-whipping&#8221; forms&#8212;automation will just execute that chaos at light speed.</p><p>Here is the practical, boots-on-the-ground guide to fixing your governance <em>before</em> you turn the robots on.</p><div><hr></div><h3>Phase 1: The Teardown (Before You Automate)</h3><p>You can&#8217;t code your way out of a process problem. Sensors drift or contradict each other. Humans improvise, shortcut, override, and forget. Context is fragmented across systems, roles, and time.</p><p>When AI fails here, we blame the model. <strong>That&#8217;s a mistake.</strong></p><blockquote><p>Most AI failures in operations are not intelligence failures. They are context and governance failures&#8212;made visible by automation.</p></blockquote><h3><strong>Why &#8220;Smarter Models&#8221; Don&#8217;t Fix This</strong></h3><p>Large Language Models (LLMs) and modern AI systems are powerful pattern engines, but they are not grounded observers of the physical world. They have no lived experience, no direct perception, and no internal world model. They rely entirely on what we feed them.</p><p>This creates three structural limitations:</p><ol><li><p><strong>No Intrinsic Grounding:</strong> AI cannot verify reality. It cannot &#8220;look outside.&#8221; It only infers from inputs.</p></li><li><p><strong>Context Fragility:</strong> Too little context leads to hallucination. AI is then asked to reason across them without a shared frame of reference. That&#8217;s not intelligence&#8212;that&#8217;s improvisation.</p><p><strong>The Edge is where mistakes become irreversible.</strong> Operational failures near the edge behave differently than failures in the center. A missed dashboard update is recoverable. A missed delivery or a wrong safety inspection is not.</p><p>The closer you get to the edge&#8212;where humans, machines, and incomplete information meet&#8212;the more costly mistakes become. <br></p><p>This is why autonomy in field operations is not primarily an AI problem. <strong>It is a governance problem</strong></p><p></p><p></p><h3><strong>The Solution: Digital Twins as Reality Interfaces</strong></h3><p>Digital twins are often misunderstood as fancy 3D simulations. That&#8217;s a shallow view. Their real value is acting as <strong>Reality Interfaces</strong>.</p><p>A proper digital twin:</p><ul><li><p><strong>Continuously ingests signals</strong> from the physical world.</p></li><li><p><strong>Aligns signals</strong> with business structures and constraints.</p></li><li><p><strong>Maintains temporal awareness</strong> (knowing what data is stale, delayed, or missing).</p></li><li><p><strong>Preserves uncertainty</strong> instead of flattening it.</p></li></ul><p>Most importantly, it gives AI something rare: <strong>A coherent, evolving representation of reality.</strong> Not a perfect one, but a <em>governed</em> one. This is how you stop pretending the map is the territory&#8212;while still using maps effectively.</p><h3><strong>Revisiting the OODA Loop</strong></h3><p>The OODA loop&#8212;<strong>Observe, Orient, Decide, Act</strong>&#8212;remains relevant for a reason.</p><p>Most AI effort focuses on <em>Decide</em> and <em>Act</em>. But real failures happen earlier.</p><p><strong>Orientation</strong> is where reality is interpreted. If your orientation is wrong, every downstream action is wrong&#8212;no matter how &#8220;intelligent&#8221; the model appears. Orientation is built from sensed reality, historical context, human judgment, and organizational constraints.</p><p>AI does not replace this. It amplifies whatever you give it.</p><p>If you optimize for clean dashboards instead of messy truth, you don&#8217;t get intelligence. Workflows are digitized, sensors are installed, algorithms optimize routes and schedules, and dashboards promise &#8220;real&#8209;time&#8221; control. In reality, the moment automation meets the physical world, uncertainty reasserts itself. </p><p>The routes cross roads with traffic and weather, sensors drift out of calibration, and human supervisors quietly improvise around rigid logic. As I wrote in my <a href="https://www.ruslantrifonov.com/p/from-chaos-to-control-why-the-future">last post about field operations</a>, <em>chaos is not a bug</em> &#8211; it is the nature of systems that involve people, weather, traffic, customers and regulators. Vendors point to demand forecasting, route optimization and &#8220;human&#8209;in&#8209;the&#8209;loop&#8221; interfaces. There is real value here. For example, logistics analysts note that companies improved demand forecasts in 2025 by combining external signals (weather, sports schedules, local events, social sentiment) with store&#8209;level inventory data. AI&#8209;assisted routing engines generated alternate transport scenarios faster than human planners, especially during port congestion or road closures. Visibility platforms using predictive ETA models and anomaly detection filtered false alarms, clustered related delays and highlighted late&#8209;stage risks. In short, AI helped <strong>surface uncertainty sooner</strong> and <strong>compress decision cycles</strong>.</p><p>AI did <strong>not</strong> eliminated surprises. Exception volumes dropped because thresholds were better aligned with operational reality, but AI did not prevent the underlying variability in demand, traffic, weather or human behavior. Multi&#8209;agent pilots suggested targeted inventory moves across distribution centers, yet planners still made final decisions. The most reliable gains came from small, well&#8209;defined bottlenecks. Automation changed who handled the chaos and when, but not whether the chaos existed.</p><h2>Pushing chaos to the edges</h2><p>When an automated system runs, the messy parts don&#8217;t vanish &#8212; they migrate to the edges of the system. These edges are where sensors meet reality, where humans improvise around rigid models, and where policies are tested against infinite edge cases. Consider the boom in robotaxis. Analyst Phil Fersht notes that robotaxis require us to surrender control over life&#8209;and&#8209;death decisions at scale. Waymo and Baidu vehicles have logged millions of miles, yet they still miss school buses or cats and collide with pedestrians. These failures aren&#8217;t because the AI can&#8217;t drive in controlled environments; they occur at the edges, where sensors encounter unmodeled situations and society hasn&#8217;t agreed who is liable when algorithms get it wrong.</p><p>Even the designers of autonomy acknowledge this. At the 2025 AI &amp; Autonomy Summit, DARPA program manager Phillip Smith remarked that &#8220;machines are supposed to be serving humans, and <strong>humans don&#8217;t even know what they want &#8212; that&#8217;s a really hard thing</strong>&#8221;. The problem is not that software lacks rules; it&#8217;s that human intent is vague, context&#8209;dependent, and often contradictory. Automation pushes complexity to the points where intent meets execution. When sensors detect only part of a situation, when policies assume edge cases away, or when humans override AI recommendations, the chaos reappears &#8212; just outside the scope of the algorithm.</p><h2>The trust and accountability paradox</h2><p>Robotaxis highlight another dimension of relocated chaos: <em>societal trust</em>. Despite millions of autonomous miles driven, consumers remain hesitant to entrust their lives to algorithms. The technology is improving, but accountability is unresolved. Robotaxi providers operate in a regulatory patchwork where state and federal rules conflict. When accidents occur &#8212; a cat killed, a school bus passed illegally, a pedestrian struck &#8212; no one knows whether the liability lies with the manufacturer, the city that approved the route, or the passenger who chose to ride. As Fersht observes, the technology is moving faster than society&#8217;s ability to adapt, regulate or trust it. Both the U.S. and China wrestle with the trade&#8209;off between <strong>scale and trust</strong>. Scaling quickly without trust is dangerous; building trust without scale is pointless. The paradox is that society tolerates human drivers&#8217; mistakes because we understand them and feel we can intervene. With robots, there is no negotiation or eye contact &#8212; just silent execution of code. Trust, like chaos, has been pushed to the edge.</p><h2>When AI failures are really governance failures</h2><p>Research on enterprise AI deployments in 2025 was damning. </p><ul><li><p><strong>An ISACA analysis</strong> found that the biggest AI failures of 2025 were not technical but <strong>organizational</strong>: weak controls, unclear ownership and misplaced trust. The authors argued that success in 2026 requires strengthening how we <strong>plan, govern and deploy</strong> these systems. </p></li><li><p><strong>A meta&#8209;analysis</strong> of AI initiatives noted that more than 80 % of AI projects failed, not because models were inadequate but because leaders treated AI as a &#8220;model problem&#8221; rather than a foundation problem. </p></li><li><p><strong>Forrester</strong> reported that only 15 % of AI decision&#8209;makers saw an EBITDA lift from AI. The pattern that separated winners from losers was simple: those that succeeded invested heavily in <strong>data readiness, governance, metadata quality and semantic clarity</strong>. Many allocated 50&#8211;70 % of their AI budgets to these foundations.</p></li></ul><p>In other words, what we call &#8220;AI failures&#8221; are often governance failures that technology exposes. The technology works as designed; it simply reveals ambiguities, conflicting incentives and missing context. The human&#8209;machine field operations matrix in my last post showed that as we move from human to hybrid to machine field agents, the tolerance for informal rules collapses. Humans can improvise around ambiguity; machines cannot. When rules are implicit or contradictory, machines execute them literally, stall on ambiguities and force organizations to confront uncomfortable accountability questions.</p><h2>Autonomy in logistics: micro&#8209;wins and macro limits</h2><p>The logistics sector illustrates how automation relocates chaos rather than removing it. AI pilots have delivered measurable improvements: better demand forecasting through signal expansion, AI&#8209;assisted routing that reduces planner workload during disruptions, and document&#8209;intelligence systems that accelerate customs and compliance workflows. Exception identification systems reduce noise by filtering false alarms and clustering related delays. These are real, valuable wins.</p><p>Yet none of these systems operate in isolation. They rely on clean data, clear policies and human judgment. Forecasts improved because teams included weather, sports schedules and social sentiment &#8212; context that the algorithms alone could not infer. Routing engines still require humans to choose among AI&#8209;generated options, and their performance depends on up&#8209;to&#8209;date traffic and weather feeds. Visibility platforms reduce false alarms by aligning alerts with operational thresholds, which is itself a governance decision about what constitutes an exception. In 2026, analysts expect AI capabilities to be embedded directly into transportation management systems and warehouse management systems, with tools dynamically weighting service, cost and emissions. They also expect <strong>context&#8209;retention protocols</strong> (like the Model Context Protocol) and <strong>graph RAG</strong> techniques to maintain continuity and understand relationship&#8209;rich data. These trends reinforce the point: as automation scales, the importance of <strong>context, boundaries and governance</strong> increases, not decreases.</p><h2>From chaos to governed chaos</h2><p>If automation relocates chaos to the edges, then governance must relocate <em>order</em> to those edges. The lessons from 2025 point to a few principles:</p><ol><li><p><strong>Start with outcomes, not experiments.</strong> Define the business result you need and assign a named owner. Don&#8217;t deploy AI for its own sake.</p></li><li><p><strong>Invest in foundations.</strong> Data readiness, metadata quality, and semantic clarity determine whether AI systems can interpret context. Allocate budgets accordingly.</p></li><li><p><strong>Make governance explicit.</strong> Keep an inventory of every model, agent and automation, and govern them with standards and approvals. Review what each system can do, where it can act and who might be affected.</p></li><li><p><strong>Design for edge cases.</strong> Assume sensors will fail and human improvisation will occur. Model handoffs between machine and human agents. Ask yourself: if a machine followed our current rules perfectly, would we be comfortable with the outcome? If the answer is &#8220;it depends,&#8221; governance is not ready.</p></li><li><p><strong>Share responsibility.</strong> AI requires cross&#8209;functional stewardship &#8212; business, technology, risk and communications all need roles. Include third&#8209;party risk reviews in every AI purchase.</p></li><li><p><strong>Build resilience.</strong> Detect problems early, communicate what happened and fix issues quickly. Capture near misses and update processes to prevent repeat failures.</p></li></ol><p>The future of automation and autonomous systems is not about eliminating chaos. It is about <strong>governing how chaos expresses itself</strong>. Machines will continue to execute rules literally, expose hidden gaps and demand explicit boundaries. Autonomous vehicles will drive, drones will inspect, and AI agents will propose procurement strategies. But unless organizations invest in governance and context, automation will simply shift uncertainty to the edges, where the consequences are more visible and more dangerous.</p><p>Chaos can&#8217;t be eradicated. It can be <strong>channeled</strong>. Automation relocates it; governance must accompany it. Organizations that learn this lesson will harness AI&#8217;s power without being blindsided by its edges. Those that don&#8217;t will find that the hardest part of automation was never the technology &#8212; it was the leadership debt they ignored.</p><blockquote><p>Humans absorb ambiguity.<br>Machines surface it.<br>Autonomous systems punish it.</p></blockquote><p></p><div><hr></div><p></p><h1>Appendix 1: Where does chaos live in your system?</h1><p></p><blockquote><p>Before adding more automation or AI, ask yourself where chaos currently lives in your operations.</p></blockquote><p></p><p>Ask these questions honestly.</p><h4>1. They try to optimize routes, digitize forms, deploy dashboards, or &#8220;add AI&#8221; &#8212; and then wonder why chaos stubbornly persists.</p><p>The real issue is simpler and harder at the same time: <strong>governance</strong>.</p><div><hr></div><h2>Field operations are already autonomous systems</h2><p>We often speak about &#8220;autonomous systems&#8221; as if they are something new. In truth, field operations have always been autonomous.</p><p>Every field agent makes decisions:</p><ul><li><p>How strictly to follow the route</p></li><li><p>How to handle an unexpected customer request</p></li><li><p>Whether to skip, delay, or reorder visits</p></li><li><p>How to trade speed against quality or safety</p></li></ul><p>These decisions are made continuously, in environments the organization cannot fully observe or control in real time.</p><p>The difference between a human field agent and a machine agent is not autonomy.<br>It is accountability, predictability, and how decisions are governed.</p><div><hr></div><h2>Chaos is inevitable &#8212; ungoverned autonomy is not</h2><p>Field operations involve many real-world participants:</p><ul><li><p>Field agents</p></li><li><p>Customers and sites</p></li><li><p>Supervisors and managers</p></li><li><p>Internal departments (sales, finance, logistics, support)</p></li><li><p>External actors (regulators, municipalities, traffic, weather)</p></li></ul><p>Each participant brings intentions, constraints, and incentives. When these collide, chaos emerges naturally.</p><p>Trying to eliminate chaos is futile. The real question is:</p><p><strong>How does an organization govern behavior when plans meet reality?</strong></p><p>This is where governance enters &#8212; not as bureaucracy, but as a coordination mechanism under uncertainty.</p><div><hr></div><h2>Governance is how intent survives contact with reality</h2><p>At its core, governance is simple:</p><p><strong>Governance is the system that translates organizational intent into constrained, observable, and correctable behavior in the field.</strong></p><p>Good governance does three things:</p><ol><li><p>Defines what <em>should</em> happen</p></li><li><p>Sets boundaries for what <em>may</em> happen</p></li><li><p>Makes it possible to see and correct what <em>did</em> happen</p></li></ol><p>Importantly, governance exists even when it&#8217;s informal.<br>The only real question is whether it is <strong>explicit and scalable</strong>.</p><div><hr></div><h2>Three governance states in field operations</h2><p>Most organizations fall into one of three states &#8212; whether they realize it or not.</p><h3>1. Ungoverned (implicit, reactive)</h3><p>Rules live in people&#8217;s heads.<br>Success depends on individual experience and goodwill.</p><p>Typical signs:</p><ul><li><p>Heavy reliance on calls, chats, and verbal coordination</p></li><li><p>Firefighting as a normal operating mode</p></li><li><p>Problems discovered after damage is done</p></li></ul><p>This model can work at small scale &#8212; and then quietly collapses.</p><div><hr></div><h3>2.
